Participant Use Agreement

 

I hereby give consent for my child/children to participate in the activities taking place at the CYO Cardinal Spellman Center located at St Joseph Seminary. My child is in good physical health condition and is physically able to participate or compete in all forms of physical activity. I know of no physical restriction whatsoever which would prohibit my child’s participation or competition in the Activity that I have selected.  I understand that there is a risk of injury to my child/children as a participant in any physical activity, and I hereby assume the risk of my child/children's participation in such activities taking place at the CYO Cardinal Spellman Center. In consideration of my child/children's participation of activities taking place at the CYO Cardinal Spellman Center/St Joseph Seminary, and to the extent permitted by law, I hereby agree to release and hold harmless the Archdiocese of New York, the Archbishop of the Archdiocese of New York his Eminence, Timothy Cardinal Dolan, Catholic Charities of the Archdiocese of New York, Catholic Charities Community Services, Catholic Youth Organization, St Joseph Seminary, its parents and affiliates, and their respective trustees, directors, officers, employees, servants and volunteers from any and all responsibility, liability, claims, and/or demands arising out of my child/children's participation, specifically including any injury that may occur due to their negligence. 

The CYO Spellman Center is on the grounds of St Joseph Seminary, an active religious institution. We are all guests here and must conduct ourselves with the utmost respect for the property, the grounds and each other. My checking the box below you agree and that you will abide by the CYO CODE OF CONDUCT. You also agree, that should any conduct not meet these standards, you or your child will be asked to leave the premises. The guiding principle behind the enforcement of this code is that the behavior of everyone involved should not detract from the children’s enjoyment of the sport/activity.

The CYO Participant usage fee is due in full prior to attending any registered session/participation taking place at the CYO Cardinal Spellman Center. Fees may be paid online and in person before the start of any participation. Once the session has started, your child/children are registered for, no refunds will be given. Sessions can not be prorated or refunded for any days or time missed from the scheduled session attending.
